So Name-Calling and Ad hominem attacks are the foundation of how to interact here? Without saying which layers are ok and which not, this doesn't make sense at all... unless this is a cheeky way of saying everything is allowed?

The lower portions are considered lower quality/value. Maybe a pyramid is not the best representation, since it's associated in the US with the 'food pyramid' where the layers have the meaning you're implying. A ladder might make more intuitive sense that high = good

By who? This is just a picture with several different types of debate. I'd say various styles of debate may be useful for different context. Sometimes "reponding to tone" is better. "Refutation" sometimes gives time and attention to a post that shouldn't be given that much highlight. These are just different styles of managing a debate, not a pyramid from good to bad.

You can converse at any level of the pyramid but repeated dragging the conversation down the pyramid can result in action against your account.
