Some fascist governments were the result of other means of political manipulation, other than a violent overthrow, but where violence has been used to topple a government, the result has often been for an authoritarian government that has had fascist-style oppression: events leading to the founding of ancient governments, dynastic Egypt, imperial Rome, imperial China, Byzantium, Mansa Musa's Mali regime, Timur's regime, Mughal India, Napoleon's regime, Mussolini regime, Stalin's regime, Hitler's 3rd Reich, and these governments.

1) Authoriitarian and fascist governments have been normally taken over by force, political manipulation (rather than popular vote), violence, by removing the opposition candidates, by voter coercion and by vote manipulation (not exactly "voted in" democratically). There is a lot of material online about this, unless you have a political history book handy.

2) The US colonies did overthrow their authoritarian colonizer, but thereafter were NOT "fascist". They formed a representative democracy that kept the power among the elite, which was not authoritarian, though it was somewhat oligarchical.
