It's a complicated gesture that means different things in different cultures.

Ronnie James Dio was the one who brought it to heavy metal culture, and to him it had been the sign his Italian grandmother used to ward off the evil eye.

I think he probably lied about that, but I agree that it means different things to different people. To most people it just means "rock on", regardless of what it really meant to the artist.
