IIRC, it's some post-surgery support thing, adopted or appropriated by troons as a sort of mascot. The most notable example that I can remember is some guy who boasted about his bottom surgery, showed his little shark plushie in picture of himself during recovery, and then proceeded to lose his fucking mind when he finally clued in that his neo-twat was just shallow, nasty meat pocket full of never-ending crud and shitstink.

Jamie Lee Curtis is his adoptive mother, if that's who you're referring to. I suspect this guy has had some trauma in his past. He's obviously severely dysfunctional.

I have some empathy for parents like Jamie because I know they're under extreme pressure to 'support' their kids or else they're going to be monstered by all of society (especially in Hollywood) and blamed for their kid's eventual suicide. She has to see that this is warped behaviour though.

Did you see how his hand is on his mom's ass?

The body language speaks volumes. JLC and her daughter are embracing eachother comfortably, but there's no physical contact with Ruby. Curtis' hand is on her leg like she's trying to keep away from him as much as possibl; JLCs body is turned towards her daughter and away from her perv son. He almost looks like he was photoshopped in.

That's what I see too. JLC's body is very turned away from him. Unless she just happened to be scratching an itch on her left leg with her right hand in the instant the photo was taken, she almost looks like she's trying to block out his presence. Her shoulder is out in front of him and she's curling away. The daughter's eyes look almost baffled too. Like she's uncomfortable, embarrassed but trying to 'be kind.'
