Can someone please explain what this means in pre-2016 English?

Are they saying doctors are being racist and providing unequal care to patients?

Or that because someone hears a racist slur will have their health affected because of it?

I honestly don't see how these things are connected, or why every state is making this same declaration. What am I missing?
