There's no forgiveness or 'do-overs' for moronic acts that endanger or end ones continued existence.

Forgiveness is always an option.

In this case, it's up to those unfortunate people who suffer from self-induced injuries.

They still have an opportunity to forgive themselves.

They still have an opportunity to ask for forgiveness from those who tried to help them, but were rejected.

Confront the demons. Rise above it.

Then atone. Rescue others who are lost inside themselves.
Show them how to find the way forward; when they're ready..

LPT.

I meant 'forgiveness' in the sense of forgiving a loan, where the need for repayment is canceled and wiped clean.

Once you cut your tits or dick off, it's pretty hard to revert to the 'before' state. Those malfunctioning people can acknowledge what they've done to themselves, but they can't restore their missing parts.

They still have an opportunity to ask for forgiveness from those who tried to help them, but were rejected.

They made their decision. Are they now saying they're not qualified to make decisions about their own life? Sounds like yet another episode in a continuing series of self-dishonorments.

Agreed. Dismemberment is irreversible.
