Who is in there saying they will track IP addresses? Should I use a VM or something to fill out this form?

TRAs love to doxx people so I highly suggest you use a VPN

Given the unpopularity of our sub's opinions, maybe people here might even benefit from a general "internet safety" post, so as to avoid anyone getting doxxed...

A VPN from a reputable provider will be enough. As long as the people making the form don't work for the VPN provider (and even then), you're safe. And if they're reputable they're not storing logs for employees to snoop anyways. NordVPN is a good recommendation, as is TunnelBear despite their current owners.

If you're worried about the site admins knowing your IP, yes. I'm not personally, but that's up to you.

The form is hosted in the UK so if it was tracking IP addresses the organisers would be bankrupt as they haven't got the required GDPR notice up.

But... I would also use a VPN

what about duckduckgo? Would that work as well as VPN?

If you're on mobile, I'd recommend the DuckDuckGo Browser(app) paired with Orbot(VPN). Or use the Tor browser instead on both mobile/computer as that has a built-in free VPN.
