I think part of it is that most people would agree that, say, getting people hooked on opioids for profit is bad, but the gender transition stuff is presented as being an imperative for people with a debilitating condition, as well as a civil rights issue of nondiscrimination in health care, and most people just kinda shrug at it because they assume it's just an adult making their own personal choice. If it were spelled out in plain terms how they are treating patients as guinnea pigs for experimental surgeries with high risks of complications and little recourse and giving children chemical castration drugs that are already known to be harmful to them and hiding behind "rights" in order to do it, they would lose public support nearly overnight. (I suspect the devout TRAs supporters would take longer to come around, they are so indoctrinated) I think the key is just awareness. When the opioid epidemic whistleblowing came out, the American public was horrified. For this, it's just a matter of time until the public realizes what's going on behind the scenes, and it's gonna take years before the story breaks in the US and the class action lawsuit start rolling in
