We all experience things that are not congruent with social norms. The well adjusted among us will admit it to ourselves but are also smart enough to keep it to ourselves.

EDIT: Also, the title is misleading. He was not "fantasizing" about his mother, but rather the ointment smell brought back an incongruent memory of his mother. So, he was in an unfortunate situation where he's applying ointment to his Johnson while being reminded of his mother. That's really not a "fantasy".
