Somewhat odd that. I've a theory that because Iran was kinda liberal and modern in a way before the fall of the shaw and the rise of the islamist government that kind of slid through the medical system into acceptance there since it was trendy in Europe towards the beginning of the 20th century. Using it as gay conversion surgery just seems like the logical outcropping of that. This guy is gay? Well that's because he's actually a woman of course!

Iran wasn't liberal and modern before the revolution. They had a few rich people who could travel and lead an international lifestyle but 99% were religious fundamentalists. That's why as soon as the elite were removed they took over. However they likely had a culture of dressing up boys as girls and raping them like Afghanistan which they could medicalise as the boys being born in the wrong body.
