What do you think about Transmedicalism? I think this is the only Trans ideology that makes sense. A person would have to fully biologically transition.

We need to stop using the word 'gender', and absolutely reject any talk of transition due to not conforming to stereotypes. Being gender-nonconforming is perfectly acceptable to most people. We certainly shouldn't be permanently modifying kids based on preferences for clothing/toys that will almost certainly change over time.

Shouldn't 'body dysmorphia' cover those who are convinced that the only path to happiness is extreme body modification?...

It's a bit more than dysmorphia. It can't be a coincidence when the "dysmorphia" affects all sexual characteristics, and it also extends to hormonal and mental functions.

Does it matter tho? If the treatment for somebody who wants to live without arms is not cutting his arms, why is the treatment for someone who wants to become a girl cutting his dick?

As a last resort, sometimes the only option is amputating his arms. I also think that while loss of the arms is a huge impairment, loss of the dick isn't really.
