Makeup would go a long way. When we see someone with makeup on we are conditioned to assume they are female. This guy does not have very masculin features, so makeup should do the trick. And really big earings, so they are immediately noticeable, definitely not studs.

That's actually some pretty solid advice lol. His hair is still a pretty gender neutral cut also, needs to be styled and if he goes full bangs it'll hid the fact that his hairline is higher than a woman's would be
