Not a surprise, that's the moment that they realize that surgery isn't (depending on what kind they are) a magical cure for their mental illness or their fetish made real; they're not women or men, they're just mutilated caricatures. At the same, they also realize that now they're in it for life. Unlike the titty skittles or roid poppers that they (mistakenly) believe they can stop with no ill consequences, the surgery is permanent.

So now they're sitting around with broken dreams and a destroyed body, it's no surprise that necking themselves is the most appealing prospect left to them.
