I think it's both. He made it up as part of his fetish. You see similar when people like this talk about having to use the men's toilets. Just the other day there was some thread on reddit, where he was like "If I have to use the men's changing rooms, they're going to pass me around like a fleshlight". That's not how someone who's afraid of that happening talks, that's the language of someone who secretly wants it to happen
