WRT the Boston Bomber witch-hunt, Reddit didn't get anyone killed, thankfully. The guy they misidentified as the bomber went missing a month before the bombing and died long before that.

Still, it must have been hell for the guy's family, who not only were missing a son for a month, but were then getting threatening messages from Internet strangers.
