It's likely a combination of crappy engineering and faulty electrical wiring at the restaurant.

Definitely not a faulty electrical wiring.

Smartphone chargers are class 2 power supplies, that is theres more than one layer of isolation between the mains and the secondary (USB), the wingdings of the transformers of these high frequency power supplies are wound one over another, in a complaint class 2 supply there would be two layers of isolation (electrical tape basically) between the two windings, of course the cheap chargers don´t even have one layer, they only rely on the very very thin coating of the enameled wires in the windings, which are not rated for mains voltage to begin with.

Still it should not shock you to death unless you have a phone with a metal casing.
