This one just struck me as depressing.

He probably went over that bump so many times with that forklift and nothing came of it. Then one day, out of nowhere, it happens to pitch forward and crush him.

No. Probably this was the first time that he was in a rush and pulled it without lowering the forks. It was bound to happen and it would happen every time as it's weight is up way too high and these things tip very easily that's why one needs extensive training to handle it safely.
