In general most doctors don't work for the hospital and are billed separately.

Being billed separately isn't the issue. Finding that the doctor who practices locally and has privileges at the hospital you go to is out-of-network by design is.

Maybe even more basically the issue is the membership in the dysfunctional system in the first place.
