Almost everyone who gets the surgery regain the weight.

The doctor should have denied his surgery when he didn't cut down by 40 lbs. As someone who lost 55 lbs I can tell you it's really easy to lose 20 lbs initially with simple diet changes. The more you weigh the easier it is to lose weight, up until a certain point. He literally would just need to eat 2500 calories a day and would lose about 2 lbs a week at his weight. It takes something like 3500 calories to maintain his weight.

Considering he couldn't lose 40 lbs for his surgery, he was never going to keep the weight off.

The thing we need to really be concerned about is how fast he gained back the weight he lost. This surgery takes awhile to regain weight. His stomach physically couldn't eat certain foods for the first year or so, but he managed to go back to his normal eating habits without fail despite losing around half of his stomach. I knew someone who got that surgery and she couldn't eat a lot of foods for the first year and a half or else she'd throw up or bust her stomach open.

This surgery forces him to stay on a diet by limiting the amount of food it can take in safely and he still managed to gain weight back. I cannot stress this enough, it's really easy to cut weight. Don't over eat and moderately exercise. I lost 55 lbs mostly by not overeating. Granted, I was only 235, no where near his size.
