Lack of testing. Lack of rapid transportation. Lack of heart problems.

It's almost like if the disease can't be quickly spread and the majority of the population isn't old or have health problems, then miraculously, the disease doesn't seem that lethal.

It is because they are not testing obviously. Cumulative testing and cases rates correlate perfectly, of course they do. To see something interesting look at the positive cases per test rates over time and you will see how that correlates with different batches of tests.

It is called herd immunity (the natural one).
Their immune systems get a lot of training.
In some of those countries they also use a lot of quinine-variants, like Hydroxychloroquine.
