Bayer sold a medicine tainted with HIV. Once it was discovered in the US, they continued to sell it overseas for over a year in order to clear stock.

How does this happen exactly? That seems fantastically unlikely to have 'accidentally' happen.

Did you read the article?

It happened in the early 80ies. At that time not much was known about HIV.

They knew enough to understand that it wasn't safe for use in the US.
The knew people would be harmed, and probably die.
