Two pharmacies in Virginia botched Pfizer doses given to children. In one case they got too much, the other, too little.

The children who were underdosed are being told to go back and get another shot (which unless I'm mistaken, will result in them ending up overdosed).

Pfizer's dilution protocol is fiddly, and I suspect this is why these errors are happening. I also suspect overdosing and underdosing happens quite frequently with adults, too, but is more likely to be swept under the rug.
