was there an obvious mistake/OSHA violation here or just a freak accident?

I use this power equipment for work. It's called a walkie stacker and it's mainly used to shoot pallets up the top steel for storage and bring them down when you need it. Normally the walkie stacker can bring pallets up and down like paper because it's power based (build to handle heavy weights).

What wrong here was the pallet the guy was shooting up (I believe) was too heavy. If you look carefully at the pallet was loaded too much of merchandise on it. Not to mention having something that heavy up high will be unstable. Resulting in the machine to tip over because it's too much weight.

Rule of advise: If you see someone using a walkie stacker get away at least 30 feet from them.

What caused it to topple over like that?

Wrong placement. More weight on the side.

It’s too much weight for a pallet jack. Should be using a fork lift. It’s baffling that no one seems to care on the floor and everyone is just walking around like nothing.
