At the 39 second mark right after the driver of the machine feels the unnatural bump, he jumps down to check if his fears just became reality. Upon seeing the crushed body and the exploded brain matter of a woman, the driver immediately gets crushed by guilt. The man will have to live with this guilt for his entire life, but until when will he be able to bear this weight?

No question it'll haunt him, but I hope he gets beyond guilt. From the look of it he might not have seen her even if he did double-check behind; she came awful close.
