I am not certain about the validity of this claim, but it may be very interesting to look deeper into,
Does Scotland actually have FOI?

The amount of official report severe side-effects used to be just 1% of the actual number. (via TheHighwire)
From this FOIR it may appear that the official reports are still a small fraction.
