Look it's really simple.

Cleanliness is nice because nobody else wants to deal with your mess. You bathe so your odor doesn't offend the people around you. Vague cultural gatekeeping mechanism my ass.

There was a girl in college who always smelled of cat piss. Everyone called her cat piss girl, we didn't even remember her name. Now it was patently obvious to everyone about that cat piss girl had some kind of mental difficulty and that was sad, but she still smelled so bad nobody wanted to be her friend.

If society, namely if the culture was far more insistent that she bathe and not smell like cat piss, she'd have friends, and she'd be better off since they'd, shall we say, provide a stabilizing effect on whatever mental illness she was suffering from. But seeing as she was offensive to everyone's noses she'd be finding herself sealing with whatever shit life threw at her alone.
