(why it was such an insulting suggestion to practice basic hygiene is anyone's guess).
That was apparently said- the reason basic hygiene was so insulting was the belief "being a doctor makes you a gentleman, and a gentleman's hands are always clean- so therefore, making a doctor wash his hands is claiming he's not a gentleman."
