There's a story reason for that. She idolised a specific hero who's a male to the point where she wanted to be treated and referred to as a man. When her father first introduced her as a daughter, she proclaimed that she's not a woman and her father went "yes fine you're the son".

I never watched One Piece beyond Impel Down ark (and when I did there wasn't yet much more beyond it. Or maybe it still lasted, can't remember), so when relatively recently I found some mentions of Yamato I was confused as fuck, to be honest. Because looks like a girl. Dresses in a way that does NOT make her look not like a girl. People say she's a dude. Wut?

Apparently, there’s a manga panel where she referred to herself as “Kaido’s son.” Some people misinterpreted the panel and thought it meant that she identified as a man, which is why there’s “controversy” around her gender. In actuality, she idolizes a certain male character to the point where she wants to be like him.
