I think they detach them then make a hole and stitch them back on, because they remove so much flesh they'd end up too low on the chest. There is a way of keeping the ducts attached which sometimes works but if you are a transman screaming that you hate being a woman and will kill yourself if you don't become a man then doctors are unlikely consider because the breast tissue could grow back.

Also it's a little ridiculous to blame doctors for not reading the mind of you in the future.

If you remove some part of your body via surgery, it's generally understood you won't be using it again.
