Star Trek has always been a progressive show, since it first went on the air in the 60s. It featured a black woman in a position of authority, which was almost unheard of back then. It had one of the first interracial kisses shown on TV, which was considered scandalous at a time when black people were still fighting for equal rights.

A lot of people at the time hated it, and yet it thrived. Honestly, in a futuristic world with advanced medicine where an actual sex change might be feasible, trans people make a lot more sense.
