This is a common issue caused by non-guaranteed network messaging. It happens on the website and even saidit.net all the time if you are using a flaky wifi connection or a flaky proxy, like with TorBrowser.

You load the page, then eventually your network connectivity gets spotty. You lick upvote and the website's JavaScript or the app's Java event sends a network message telling reddit of the vote...but that message silently fails. Then you do a full reload of the page and that succeeds, but only then do you learn your vote message failed.
