People start to believe their own propaganda after a while. They aim it at impressionable people to push a narrative then act surprised when those same people start repeating the narrative and believing it.

It's similar to how in politics the republicans cultivated an extreme right wing mob through dog whistles towards astroturfed tea party which started out as a legitimate protest against wall st bailouts. Then John Bohner was like surprised Pikachu face when those people ended up in Congress and blocked his pro corporate agenda because it wasn't extreme enough for them... Then fast forward and Trump starts saying the quiet part out loud and all the neocons are jumping into the democratic party and acting outraged that the base took their propaganda too seriously.

It seems very similar to the TRAs pushing a crazy narrative to move the Overton window then suddenly your community is dominated by AGPs and infinite gender ClusterBs now all the transmeds / transexuals are putting on the surprised Pikachu face and getting called truscum or whatever.
