You submit your paperwork and a team of nurses and doctors will go over it with a fine-toothed comb and send it back for revision as necessary, then send it to CDC, where another team of docs will do the same. After that, it gets logged as an official AE report for statistical purposes at CDC. They have a process to bring danger signals to their attention.

...i mean, the initial team is 50 people for the whole country, which is usually adequate until you get a couple MILLION reports, but still...

For this to be a straight-faced excuse means there is blistering blinding gobsmacking incompetence at CDC at the very best.
